Possible Cause How to Check Solution

3 Fitting nut too loose.
  1. Make sure tubing nuts are tight, but do not over tighten as this will cause collapsing of the connector nose (as in any flare fitting) which will result in tubing lines blowing off just as they do when not tightened enough.
